Cartagena is an incredible city. In fact, Colombia is an incredible country and its citizens are friendly and unassuming. Spanish is not a must, but is extremely helpful.
The old walled city allows one to wander for days, viewing incredible architecture, world class shopping and great restaurants. To get a feel for the real Colombia, visit the old city and surrounding areas. To the west (by boat) you will find an island chain that has incredible aqua marine water, perfect for snorkeling or just boating. Many tour operators run to the islands or you can do as I do and rent a boat and leisurely cruise the islands and visit a variety of beaches (Playa Grande, Rosario Islands).
